Corey Klein, PT, provides one-on-one physical therapy in patients’ homes across north Atlanta, with a clinical focus on spine rehabilitation. Referrals are welcome for patients who would benefit from focused, individualized care, especially those for whom getting to a clinic is a barrier.

Pre-operative conditioning and post-operative rehab after fusion, discectomy, laminectomy and other procedures.
Complex and chronic spine pain
Including persistent pain after surgery and patients seeking conservative care.
Orthopedic post-op
Joint replacement and rotator cuff repair recovery at home.
Balance and fall risk
Fall risk assessment, gait and balance training.
Vestibular and concussion
Certified vestibular and concussion specialist.
Neurological conditions
Parkinson's, MS, neuropathy and post-stroke weakness.

Protocols and restrictions followed as written.
An evaluation summary after the first visit.
Progress updates at the frequency you prefer.
Prompt communication about any concerns.
Every visit delivered by a licensed physical therapist.
How to refer
Call (678) 543-0699 to refer a patient. Please include the diagnosis, any surgical protocol or restrictions, relevant imaging and your preferred update schedule.
For dry needling, please include a dry needling order.
